The Open Enrollment Period for Institutionalized Individuals (OEPI) is continuous for eligible individuals who wish to enroll in Great Plains Medicare Advantage (HMO I-SNP) or Great Plains Medicare Advantage Gold (HMO I-SNP). If you are eligible, you can enroll in Great Plains Medicare Advantage at any time.
Eligibility Requirements
You can join Great Plains Medicare Advantage if you have Medicare Part A (Hospital) and Part B (Medical), and live in a participating skilled nursing facility. You must live or be expected a live in a participating skilled nursing facility, as determined by the state, for 90 days or more. Other eligibility requirements may apply.

Great Plains Medicare Advantage covers all Medicare Part A and Part B-covered services. In addition, the health plan provides Medicare Part D Prescription Drug Coverage and supplemental benefits. These supplemental benefits include services such as routine hearing and eye exams, and come at no extra cost to you.
