The Open Enrollment Period for Institutionalized Individuals (OEPI) is continuous for eligible individuals who wish to enroll in Great Plains Medicare Advantage (HMO I-SNP) or Great Plains Medicare Advantage Gold (HMO I-SNP). If you are eligible, you can enroll in Great Plains Medicare Advantage at any time.

You can join Great Plains Medicare Advantage if you have Medicare Part A (Hospital) and Part B (Medical), and live in a participating skilled nursing facility. You must live or be expected a live in a participating skilled nursing facility, as determined by the state, for 90 days or more. Other eligibility requirements may apply.

The following facilties participate in the Great Plains Medicare Advantage Institutional Special Needs Plan:

Great Plains Medicare Advantage covers all Medicare Part A and Part B-covered services. In addition, the health plan provides Medicare Part D Prescription Drug Coverage and supplemental benefits. These supplemental benefits include services such as routine hearing and eye exams, and come at no extra cost to you. Please see the Supplemental Benefits section below for more information about supplemental benefits.

If you meet the eligibility criteria, you can enroll at any time, effective the first of the following month.
